If you purchased or acquired Elanco Animal Health Inc. common stock registered with the Commission and traded under the symbol ELAN (the “Security”) during the period from May 9, 2019, through May 6, 2020, inclusive (the “Relevant Period”), you may be entitled to a Distribution Payment from the Elanco Animal Health Fair Fund.
The Elanco Animal Health Fair Fund (the “Fair Fund”) has been established to provide distributions to eligible investors. The Fair Fund is intended to benefit individuals and entities, or their lawful successors, who purchased or otherwise acquired Elanco Animal Health Inc. common stock registered with the Commission and traded under the symbol ELAN during the period of May 9, 2019, through May 6, 2020, inclusive (the “Relevant Period”). To be considered for eligibility to receive a Distribution Payment from the Fair Fund, Preliminary Claimants must submit a completed Claim Form together with the required supporting documentation. If you would like to be eligible for payment, you must submit a completed Claim Form with the necessary documentation on or before the deadline to be considered for eligibility to receive a Distribution Payment from the Elanco Animal Health Fair Fund. If you send the Claim Form by U.S. Mail, it must be postmarked by September 14, 2026 (the “Claims Bar Date”). If you submit the Claim Form online , it must be received by September 14, 2026. Claims that are not submitted by this deadline will not be considered.
